We accept all family friendly vendors! You may pick up forms & drop off to the City Clerk's Office, 20 Hammond Street between the hours of 9 to 4.
Mail to the same address. WE ONLY ACCEPT CHECK, CASH, PMO, CASHIERS CHECKS. Payment needs to be accompanied with Application. NO C.O.D. Applications can also be found on the cities website. CHOOSE THE CORRECT FORM PLEASE.
