To make your appointment as smooth and efficient as possible, you now have the option to complete your consent form ahead of time.
• Print and fill it out at home, or
• Stop by the shop prior to your appointment to pick one up
Arriving prepared helps ensure we stay on schedule and gives you more time in the chair.
Please remember:
– A valid ID is required
– Forms must be fully completed
– A 50% deposit is required to book (non-refundable)
If you’re unable to print, forms will always be available in-shop.
We appreciate you taking the time to come in prepared.
