Even after just 1 session, the process has begun.

As a safety protocol, I keep the laser at a lower fluence during the first treatment. This allows me to see how the skin responds while minimizing unnecessary risk. Even with this conservative approach, it's common to see early changes, and this tattoo is a great example.

Look closely at the lettering and you'll notice the black ink is already starting to break apart. Areas that were once solid and dense are beginning to look grainy and less uniform. That's exactly what we want to see after a first session.

These are very encouraging results. The client's skin responded well, the treatment was tolerated successfully, and the tattoo is already moving in the right direction. As treatments continue and settings are adjusted appropriately, I expect this tattoo to continue fading nicely.

4 sessions in, and the results are just starting to get exciting.

Heavily saturated tattoos often don't show dramatic clearance during the first few treatments. The laser has to break down a tremendous amount of ink before noticeable fading becomes visible. Around the fourth session is often when the skin starts to "peek through" and progress becomes much easier to appreciate.

Look closely at this eagle tattoo and you'll see areas where the skin is beginning to show through the black ink, especially in the wings and body. The tattoo is lighter, less dense, and moving in the right direction. This client is off to a very promising start, and we expect continued fading with future treatments.

Why do underarm tattoos often remove so well?

One of the reasons tattoos in the underarm and upper inner arm area can respond exceptionally well to laser removal is their proximity to the body's axillary lymph nodes, which are a major part of the lymphatic drainage system.

When a Pico laser shatters tattoo ink into tiny particles, your body does the hard work of clearing those particles away. Much of that clearance happens through the lymphatic system. Because the underarm area has a dense network of lymphatic vessels and is located close to the axillary lymph nodes, the body can often transport and eliminate fragmented ink more efficiently than in areas farther from major lymphatic drainage pathways.

That's why tattoos on the chest, upper arm, shoulder, and underarm region frequently show impressive fading in fewer sessions compared to areas like the hands, fingers, lower legs, ankles, and feet, where circulation and lymphatic drainage tend to be less robust.

Unfortunately, this client was told that using acid would "remove" a tattoo. Instead, the acid damaged healthy tissue, created scar tissue, and left the tattoo largely untouched.

The good news? Even difficult cases like this can often be improved with the right technology and a patient, methodical approach.

These photos show the results after just 4 sessions. Not only has the tattoo significantly faded, but I've also been using fractional laser treatments to help break up and remodel the scar tissue created by the acid burn. The skin is already becoming smoother, and if healing continues as expected, there's a very good chance this area will be dramatically improved with several more treatments.
