• Official Trustee Nomination Form

• Official Proxy Form

Please download the required form, complete and sign it, then email it to:

Important deadlines:

• Trustee Nomination Forms must be received by 12:00pm (midday), Sunday 12 July 2026

• Proxy Forms must be received no later than five (5) clear days before the AGM.

Late or incomplete forms cannot be accepted.
