Client Declaration
I confirm that I have provided accurate information regarding my hair history, including any previous colouring, chemical treatments, scalp conditions, allergies, sensitivities, medications or medical conditions that may affect the fitting, maintenance or removal of hair extensions.
I understand that hair extensions require regular maintenance and aftercare to ensure the best possible results and longevity.
Client-Supplied Hair
Where I choose to supply my own hair extensions, I acknowledge and agree that:
* Studio S cannot guarantee the quality, condition, suitability, colour match, lifespan or performance of client-supplied hair.
* Studio S accepts no responsibility for shedding, tangling, matting, colour fading, damage, poor quality or any defects associated with client-supplied hair.
* No refunds or compensation will be provided for issues arising from client-supplied hair.
* Any corrective work required due to the quality or suitability of client-supplied hair will be chargeable.
Hair Extension Fitting
I understand that the fitting service is provided using professional techniques and products. While every effort is made to achieve the desired result, individual results may vary depending on hair type, condition and maintenance.
Aftercare Responsibilities
I agree to follow all aftercare advice provided by Studio S, including:
* Using recommended brushes and hair care products.
* Brushing extensions carefully and regularly.
* Keeping extension bonds, rings or fittings clean and dry.
* Sleeping with hair tied back or braided where advised.
* Avoiding excessive heat, pulling or tension on the extensions.
* Attending maintenance appointments as recommended.
Failure to follow aftercare instructions may result in damage, slippage, matting, tangling or loss of extensions.
Loss, Damage or Slippage of Extensions
I understand and agree that:
* Hair extensions may naturally loosen over time due to hair growth and daily wear.
* Failure to follow aftercare instructions may contribute to extensions slipping or becoming damaged.
* Any extensions that are lost, shed, pulled out, removed or fall out after fitting may require replacement.
Where replacement hair and refitting are required, I agree that:
* Replacement hair will be charged at the current Studio S retail price.
* Refitting and labour costs will be charged at the current Studio S fitting rate.
* No free replacement hair or fitting will be provided unless Studio S determines there has been a fitting error.
Allergic Reactions
I understand that although professional products are used, allergic reactions can occur. I accept responsibility for informing Studio S of any known allergies or sensitivities before treatment.
No Guarantee
Studio S cannot guarantee the lifespan of hair extensions due to factors outside its control, including:
* Hair type and condition.
* Home care and maintenance.
* Swimming, sun exposure and lifestyle factors.
* Use of unsuitable products.
* Client-supplied hair quality.
Refund Policy
Due to the nature of hair extension services and hygiene regulations:
* No refunds will be given once hair has been fitted.
* Any concerns must be reported within 48 hours of the fitting appointment.
* Studio S reserves the right to inspect any issues before determining an appropriate resolution.
Consent
I have read and understood this document. I have had the opportunity to ask questions and agree to proceed with the fitting of hair extensions under the terms outlined above.
