1. Pass your 𝐂𝐎𝐌𝐏𝐋𝐄𝐓𝐄 𝐃𝐎𝐂𝐔𝐌𝐄𝐍𝐓𝐒 at our official email address or hand-in your printed documents at the DA-AMAD VI office:

✅Application letter addressed to the Regional Executive Director Dennis R. Arpia;
✅Duly accomplished Application Form;
✅Business Model Canvas (BMC) with Activity & Financial Plan as prescribed by the program;
✅Clear scanned copy of at least one (1) Valid ID (indicating date of birth);
✅Proof of Residency from any of the following:
- Barangay Certificate
- Valid ID indicating home address
✅Endorsement letter from the Provincial / Municipal / City Agriculture or Veterinary Office
✅Latest 2x2 ID Photo with White Background
✅Notarized Authority to Use of Land (for release of fund)
✅Needed additional documents per category:
- Processing - Duly accomplished Form 2 - Suppliers Information
- Digital Agriculture - Blueprint, Autocad, among others
